Sports physicals

Before a student can play a sport, he or she must have a physical exam and parent permission. Both are required by state law.

 

All students participating in interscholastic athletics must have a physical within a year before the first day of practice. This required physical should be conducted by the student's health care provider. If a student does not have a health care provider, the school nurse can schedule a physical with the district physician.

 

Also, permission forms must be signed by a parent or guardian and turned in before the start of each sport season. This process is repeated for the fall, winter and spring sports seasons.

 

District offiicals must have two documents before the student can participate in sports:

  • A health appraisal form completed by the student's health care provider. The district form is called Student Health Appraisal HE-104 (click on the highlighted text to download one), but many doctor's offices have their own version of the form. Either is sufficient.

  • A health history and permission form to be completed by the student's parent or guardian. This form is called Seasonal Sports Interval History HE-106 and must be filled out before the start of each season.

Completed forms should be turned in to the school nurse. In addition, the Seasonal Sports Interval History form contains a concussion information sheet that parents/guardians must review and keep for their records.

 

Forms also are available from all school nurses' offices and guidance offices and the Physical Education Office at Albany High School.

 

Finally, a student cannot play a sport until he or she and a parent attends the "Update for Interscholastic Sports" information session held before each sport season.
